Out of Network Benefits

If I am not in network with your insurance, I will provide you with a superbill (receipt of services) to submit to your insurance. Some insurance companies have a plan where they will then reimburse you for a portion of the amount of the service for an out of network provider (some do require that you meet the out of network deductible first).

Late Cancelation/No Show Policy

In order to be mindful of your time and mine, I require a 24 hour notice of cancelation for any appointment. If you are unable to cancel your appointment in advance of the 24 hour notice, there will be a $75 fee.

Fees

Please note: If you are using insurance, your fees are determined by your insurance provider and are not accurately reflected below.

Individual therapy (53-60 minutes): $200

Individual Therapy (45 minutes): $170

Family Therapy: $200

Interactive Complexity (Play Therapy Add on): $30

Insurance

I am currently credentialed with the following insurance providers:

  • Blue Cross Blue Shield of Illinois PPO

  • CAT network

  • Cigna

  • Optum/United Healthcare

  • Aetna

All plans differ. If you would like to use your insurance benefits, please call your insurance provider before your appointment to ensure that I am in network with your insurance plan. Your insurance provider can also give you an estimate for the out of pocket amount that you will be responsible for your service.
